Dental SEO or Google Ads: which should a Saudi clinic pay for first?
A guide for clinic owners to what each channel buys, with a check of four Riyadh patient searches loaded twice: the ads changed between loads, the organic results did not, and almost none of the advertisers were on organic page 1.
Google Ads buys a place above the results for as long as you pay for each click. Dental SEO builds the pages and Business Profile that Google shows without a click fee, more slowly and with no promise. In four Riyadh searches, clinics buying ads were rarely on organic page 1, so most need both, with different jobs.
This guide is for owners and managers of dental clinics in Saudi Arabia deciding where next month’s money goes. On 28 October 2026 we loaded four patient searches from a Riyadh location, twice each, and read the sponsored and organic results. We also opened the Google, Analytics and Ministry of Health pages cited below. We sell SEO, not ad management, so we have a reason to favour SEO. The checks below do not depend on us.

What each one buys
The two channels are paid for and judged in different ways, and Google decides them in different ways.
| Google Ads | Dental SEO | |
|---|---|---|
| How a patient sees you | An auction runs each time someone searches. Your bid, the quality of your ad and landing page, and the competition decide whether you show and where | Google reads your pages, your Business Profile and the places that mention you, and picks what to show |
| What you pay for | Clicks. You set a maximum per click and a daily budget; a month’s charge cannot exceed 30.4 times the average daily budget | The work, not the position. Google says no one can guarantee a first place, and that you cannot pay for a better local ranking |
| How fast | When the ad is approved and wins the auction | Google says some changes take effect in a few hours and others take several months |
| When you stop | New clicks stop | The pages and profile stay on your site, with no promise they keep their position |

The row that matters most is the last one. A click you buy is gone after it is used. A page you build is still there next month, and your treatment pages can keep answering searches after the invoice is paid.
What four Riyadh searches showed
We chose three treatments that clinics advertise: implants, braces and Hollywood smile. Google Autocomplete suggested offers and cost for all three on the same day. Three searches were in Arabic and one was in English. Each was loaded twice in the same hour with a Riyadh location code, and we recorded what appeared. We did not click any ad, because a click costs the advertiser.
| Search | Sponsored results at the top, first load then second | Organic page 1, second load |
|---|---|---|
| زراعة الاسنان الرياض (implants, Riyadh) | 4, then 3 | The same 8 results in the same order |
| تقويم الاسنان الرياض (braces, Riyadh) | 3, then 4 | The same 9 results in the same order |
| ابتسامة هوليود الرياض (Hollywood smile, Riyadh) | 1 plus a sponsored map card, then 0 | The same 9 results in the same order |
| “dental implants riyadh” (English) | 4 plus a mid-page sponsored unit, then 0 | The same 9 results in the same order, with an AI Overview in both loads |

Five things stand out.
The paid results moved and the organic results did not. Across the eight loads we counted 19 top text ads from 8 readable advertiser domains (one ad had no readable address), and 2 of the 8 loads showed no ads at all. On the braces search the first load showed three advertisers and the second four, with one in common. The organic page 1 was identical in all four pairs. An ad is a fresh auction every time. In these searches the organic page 1 did not move.
The advertisers were not the organic winners. Of the 8 advertiser domains, one appeared on organic page 1 of these searches, and only on one of them. We cannot tell why. A clinic may buy clicks because it has no organic position, or the clinics holding organic page 1 may not advertise. We did not ask them, and the data cannot say which. What it does say is that ads and organic results are different slots with different holders.
Offers lead the ads. Nine of the 19 top ads carried a discount, a price, an instalment plan or an offer in the visible text, found by word match, and we read the text of seven of the nine. We name no clinic and make no claim about any offer. The point is that a price in an ad is a health advertising question, covered below.
A directory sits first. In all four searches a booking directory held the first organic result. Of the 35 organic results on page 1 in the first loads, 7 were directories and 3 were social posts, which left 25 for clinic, hospital and other sites. Your own site competes for those 25 places. A directory is a third channel that neither ads nor SEO on your own site controls.
The map pack sat below the ads. A map pack showed in three of the four searches and came after the top ads whenever ads were present. Google says there is no way to pay for a better local ranking, so the organic pack is earned, not bought. The sponsored map card we saw on one search is labelled as an ad. Our local SEO guide covers what feeds the pack.
The limits are plain. Four searches, eight loads, one afternoon and one location code. Auctions change with the time of day, the device and the person. We do not know what any clinic spends, which of them also pays an agency, or what any of them earns.
One page should serve both
Google lists the quality of your ad and landing page among the factors in Ad Rank. That makes the treatment page the place where the two channels meet. A page that answers the search, shows the doctors and the hours, and offers a way to book helps the quality assessment of an ad and helps the page earn organic visits. An ad that lands on a home page, or on a page that does not mention the treatment searched, pays for the click and wastes it.
This is why we build a page for each treatment first, whichever channel the clinic starts with. The 90 day plan for more dental patients shows the page order we use for implants, aligners and veneers.
The arithmetic: cost per booked patient
Compare channels on one number: what each booked patient costs.
Cost per booked patient = cost per click / (enquiry rate x booking rate)
Take made-up round numbers. A click costs 10. Of every 100 visits, 5 send an enquiry by call, form or message (5%). Of every 10 enquiries, 4 book and attend (40%). One hundred clicks cost 1,000 and bring 5 enquiries and 2 booked patients, so each booked patient costs 500.
| Scenario | Enquiry rate and booking rate | Cost per booked patient |
|---|---|---|
| Base | 5% and 40% | 500 |
| Weaker page | 2.5% and 40% | 1,000 |
| Slower replies | 5% and 20% | 1,000 |
| Both | 2.5% and 20% | 2,000 |
Khaleej SEO calculation with hypothetical inputs.
The click price is 10 in every row. Two things the clinic controls, the page and how fast the phone is answered, moved the cost four times. A cheaper click cannot fix a page that does not convert, and a better ad agency cannot fix a phone that rings out.
Do the same sum for SEO. Divide the monthly fee by the booked patients who came from organic and map enquiries. The number takes longer to appear, because Google says to wait a few weeks before judging a change. Our ROI guide shows how to count the full cost, and our timeline guide shows what to expect month by month.
Pick your first move
| Your situation | Start with | Why |
|---|---|---|
| A new clinic or branch opens within two months | Ads on the one treatment you most want to sell, capped, plus the profile and that treatment page from day one | The pages need time to be found. Ads can fill the first weeks, but only if the landing page exists and calls are answered |
| The clinic is established, reviews are good, treatment pages are thin or missing | SEO first | Patients search for implants and aligners all year. Paying for clicks to a home page is the weaker-page row in the table above |
| A dated campaign: an offer, a new device, a seasonal window | Ads | SEO cannot be timed to a three week window. Check the price and offer rules first |
| Several branches | SEO: one page per branch, then ads only for a new branch | Each branch needs its own address, hours and doctors on its own page |
| You cannot count calls or tell which enquiries book | Measurement first | Neither channel can be judged without the two rates in the arithmetic |
| One budget and you need patients this month | Ads, with a cap | An approved ad can show as soon as it wins an auction. SEO runs on Google’s clocks of hours to months |
If you have more than one row that fits, start with the one that fits your slowest problem. A clinic with good reviews and no treatment pages should not buy its way around the missing pages.
What the rules say
Health advertising is regulated, and both channels sit inside the rules. This is our reading, not legal advice.
- Google’s healthcare policy. Google expects ads and their destinations to follow appropriate laws and industry standards, and expects you to know the local law for every location your ads target. The landing page counts, not just the ad.
- Google’s targeting limits. Google’s restricted targeting page lists Health among the sensitive interest categories, and says advertisers promoting products and services in those categories cannot use advertiser-curated audiences, such as lists built from your own visitors. Predefined Google audiences stay available. Whether a treatment campaign counts as Health is a question for your Google Ads contact. We did not test it. Do not plan a campaign around re-reaching your own site visitors until you have the answer.
- The Ministry of Health. On 21 April 2024 the Ministry said it had issued advertisement violations and a four month suspension to a practitioner it found continuously publishing commercial advertisements without a scientific basis. A 2014 notice says private health institutions must have their price lists approved by the Ministry, publicize the prices and not change them without its consent. So an offer that quotes a price is a compliance question, not only a design one.
Nine of the 19 top ads we saw carried offer wording. We make no claim about any of them. Before you publish a discount or a price, ask a licensed compliance contact. Our guide to choosing a medical marketing company lists the questions to ask about who reviews medical claims.
Count both channels the same way
Count three things for each channel: calls, form submissions and direction requests. In Google Analytics the recommended event for a submitted form is generate_lead. Calls and direction taps are custom events you create. If you also record which enquiries were real and which became patients, Google’s recommended events for that are qualify_lead and close_convert_lead.
The search terms report in Google Ads shows how your ads performed when triggered by actual searches. Read it as a list of what patients type. The searches that convert are the next treatment pages to build. This is the cheapest thing ads give an SEO plan.
A 90 day plan to run both
| Period | Ads | SEO |
|---|---|---|
| Days 1 to 30 | One treatment, one landing page, a capped daily budget | Complete the Business Profile, set up the three counts, build the page for the same treatment |
| Days 31 to 60 | Read the search terms report; remove searches that bring clicks and no enquiries | Build the next treatment page from the converting searches; fix hours and address everywhere |
| Days 61 to 90 | Work out cost per booked patient | Work out booked patients from organic and map enquiries; compare, then raise, hold or cut each channel |
Our clinic map troubleshooting guide covers the Business Profile fixes for the first month.
When neither is the first move
If the phone is not answered the same day, no channel will pay. If the clinic’s price lists and licences are not in order, fix that before any offer goes live. And if most patients arrive by referral and you cannot count calls, begin by counting.
